Referrals

The referral package contains a Cathedral Home application, a medical intake form, and several release forms. A checklist is located in the referral package for your conveniance. All materials in the referral package must be completed and returned to Cathedral Home. Upon receipt of the referral package you will be contacted by the appropriate person.
