  • This is a family-led program designed to help families feel supported to achieve their goals. We prioritize individualized consultation focused on resilience, protective factors, healthy relationships, and self-advocacy so families develop confidence and tools leading to ongoing positive change. Family Care Coordination services can involve one or a combination of the following: resource coordination, one-on-one parenting, life skill-building sessions, and team-based collaboration and planning to address goals.

Care Coordination is a great transition from, or alternative to inpatient treatment, for youth in foster care, juvenile legal systems, and other out-of-home placements. Families and youth are supported as they develop confidence and tools leading to ongoing positive change. Call us to get started.

  • We offer coaching and skill-building activities to support young people while strengthening their life skills as they transition into adulthood. We provide one-on-one individualized coaching to help young adults develop skills including education, employment, housing, and connection to community resources.

Services are offered to any youth who wishes to participate, with additional resources available to young adults between 14-20 years old (Chafee eligible) who have previously been in a court-ordered, out-of-home placement. Call us to get started.

  • Our curriculum is developed in collaboration with the Nurturing Parenting Programs and is grounded in evidence-based research. With a focus on fostering nurturing behaviors, promoting healthy physical and emotional development, and teaching appropriate role and developmental expectations, the program offers a wide range of activities to help parents develop their skills. Please contact us by calling (307) 742-3302 or emailing community@cathedralhome.org to get started.

  • Supporting youth and their families to prepare for transitions back into their communities from an out-of-home placement (i.e. treatment facilities or group home placements).

Through the transition phase, youth may be preparing to rejoin their family or live independently. Young people transition successfully into adult independent living by building supportive relationships, learning life skills, and seeking employment.

We connect with alumni frequently to check-in. If needed, we connect them to resources including scholarships for continuing education, and temporary financial assistance, and align them with supportive long-term resources to maximize their potential. Contact us to get started.

  • If you are an alum of Cathedral Home, there are resources and assistance available to you. We would love to support you in your pursuit of higher education, with temporary financial assistance, or provide more information about how we can support you in your community.
